These are some of the forms that the Diocese of Saskatoon uses.  All forms are in PDF and you will need to have Acrobat Reader.  The Acrobat Reader is available free from Adobe.     

The forms are also available in Word document.  You can choose to download in PDF    or click on  for the word document.  The word document is useful to fill the form online and email it to the diocese.  Click on , then click on open and the document will appear for your use.  After you have filled in the form, you can click on file, scroll down to send, click and then you can email.
