All the forms are in Word Document. Simply click on the title of the form and bring it up on your screen. You can then type in the information that you need to send to the synod office. Once you have entered in your information, save your form to some place you can remember (your desktop is a good place). Open your email and send it as an attachment.

2008 Certificate of Declaration (this form is to designate who the warden's and vestry are for the year 2008)

Certificate of Election (this form is to designate who the parish has elected to be the diocesan synod delegates)

Annual Statistics (this form is to designate the statistics for the year 2007)

Parish Officers for the Year (this form is to designate who the parish officers are for the year 2008)
